Like Mario, Luigi has had his own medical persona, known as Dr. Luigi. Dr. Luigi was first introduced in his own eShop game for the Wii U, titled Dr. Luigi, and he was also included in a mode of Dr. Mario: Miracle Cure known as Operation L. He reappeared in Dr. Mario World as one of the doctor characters who could be unlocked at random through the Staffing feature. In World, his skill in Stage Mode was to eliminate random objects in an L shape, while in Versus Mode, his skill was to prevent opponents' capsules from spinning for 5 seconds. Dr. Luigi wore his normal green hat during his original appearance, and he only wore the head mirror instead during his later appearances.

Baby Luigi was also introduced in Dr. Mario World in September 2019 as Dr. Baby Luigi. Like Dr. Luigi, he was available at random through the Staffing feature. A Fire Luigi variant of Dr. Luigi was also made available through the Staffing feature starting in March 2020.
